Sir , how to draw heel bisector line ?
Make the patient prone. Bend the knee by 90 degrees. Keep the ankle in neutral position. Draw midline axis of heel. It should pass through 3rd toe if normal.

If the quadriceps is weak it should not be done, remember that gastrocnemius is an extensor of knee due to its origin from femoral condyles. Once the foot is fixed on the ground it can pull the femoral condyles back to produce extension of knee. Langenskiold positive means that equinus is predominantly due to gastrocnemius involvement with sparing of soleus, gastrocnemius alone needs to be corrected.
